If you do not have access to Microsoft Excel, you can download OpenOffice, which is a free office suite that contains a spreadsheet editing program. OpenOffice is mostly compatible with the template, but the explanatory text which is supposed to disappear when you save the file as a PDF may not disappear. Click here to download OpenOffice.

After you download the Excel file you will need to enable editing.

IMPORTANT: Please ensure that your margins are set to Narrow and your page orientation is set to Portrait. For details on how to set these follow these links: Changing Margins, Changing Orientation.

If you are experiencing issues saving the PDF as a single portrait page, please send us the Excel file instead of an incorrectly formatted PDF.
